- Support members during 30-day post-hospital transitions.
- Role includes coordinating discharges, conducting hospital/home visits, reconciling medications, and educating members on follow-up care and resources.
Qualifications:
- Active CA RN license
- 1–3 years hospital discharge planning (3–5 yrs preferred)
- Reliable transportation for ~10% local travel in Los Angeles, CA
