 Compétences:

  • Position is fully remote, but candidates MUST be local to the 5 boroughs of New York (Bronx, Manhattan, Brooklyn, Staten Island, or Queens).
  • Candidate must be bilingual in ENGLISH/KOREAN
  • Candidate MUST have customer service skills as this position will be making outbound calls to members.
  • MUST have HS Diploma/GED
  • Minimum 1 year experience working with underserved or special needs populations, with varied health, economic and educational circumstances.

 

Résumé :
Community Connectors works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long term care, for members with high need potential. Staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
