Case Manager UAS RN (NY- Field Visits)

Salary: $60.00 Per Hour

  • Conduct home visits to perform UAS assessments.
  • Manage a caseload of at least 15 cases per week.
  • Develop and update care plans based on assessments.
  • Coordinate with the care team to address patient needs and manage transitions.

Required Experience: UAS Certified RN with a New York State license. New grads are considered; those without UAS certification but with home care experience are also considered.

Required Education: Bachelor’s degree in Social Work or Health Education, or equivalent experience.

Required Licensure/Certification: NY State RN License.
