About the Role:

  • We are looking for a Case Manager to join our team. This role will be fully remote with occasional home visits required in the Greater Columbus, OH area.
  • The expected schedule is from 8/11 to 11/7, Monday to Friday, 8AM to 5PM.
  • The primary responsibilities include telephonic and/or in-person visits with members receiving home care services, assessment of needs, authorization of appropriate services, creation and maintenance of care plans, and monitoring of services.

Responsibilities: -

  • Proactively identify members who may qualify for case management services
  • Conduct assessments of member needs by collecting information from various sources 
  • Develop and implement a collaborative care plan with the member, caregiver, and healthcare professionals
  • Monitor the effectiveness of the care plan and make modifications as needed
  • Promote integration of services for members to enhance continuity of care
  • Conduct face-to-face or home visits as required
  • Maintain department productivity and quality measures
  • Manage and complete assigned work plan objectives and projects in a timely manner
  • Attend regular staff meetings and participate in Interdisciplinary Care Team meetings

Education Qualification: -

  • RN/LSW/LISW license in Ohio
  • Minimum of 1 year of case management or managed care experience
  • Minimum of 1 year working with individuals with chronic conditions and home care supports

Required Skills: -

  • Strong assessment skills
  • Excellent organizational abilities
  • Ability to work independently
  • Comfort working with individuals with chronic conditions
