About the Role
- This is a fully remote position available to candidates residing in Kentucky. The role involves making outreach phone calls to Medicare members to support the closure of HEDIS gaps in care.
- The Health Educator will also be responsible for searching alternate phone numbers for members and documenting all phone calls. The expected work schedule is either 8-5 or 9-6 EST or CST.
Responsibilities
- Conduct outreach phone calls to Medicare members to support HEDIS gap closure.
- Search for alternate contact information for members as needed.
- Document all phone calls and interactions accurately.
- Act as a resource to health educators and assist in resolving issues during intervention protocols.
- Make administrative decisions in the absence of a director or manager.
- Design, implement, and evaluate health education interventions based on departmental priorities and contracts.
- Conduct needs assessments specific to populations, geographies, and members.
- Develop internal resources in collaboration with the Resource Specialist.
- Identify and evaluate community health education services and develop referral agreements.
- Function as a liaison to internal departments and external organizations to ensure resource information is current.
- Provide input on database design and functionality.
- Attend work group meetings for maintenance and coordination issues.
- Coordinate health education services assessments and collaborate with Quality Improvement (QI) on oversight.
- Perform other duties as assigned, including the submission of required monthly reports.
Education Qualification
- Certified Community Health Worker (CCHW) certification is required.
Required Skills
- Clinical background with at least 1 year of experience in healthcare or clinical settings.
- Strong telephonic customer service skills for member interactions.
- Proficiency with MS Office tools, including Excel and Word.
- Ability to adapt to a fast-paced environment with frequent changes in priorities.
- Experience with remote work and effective time management.
- Residency in Kentucky is required.
- Preferred experience with HEDIS.
- Required IT equipment includes a laptop, one external monitor, headset, keyboard, mouse, and HDMI cable.
