Pay Range $39/hr-$41/hr

About the Role:

• We are seeking a Case Manager to work remotely with home visits required in the Greater Columbus, OH area.
• The role involves assessing member needs, authorizing services, creating and maintaining care plans, and monitoring services.

Responsibilities: -

• Conduct telephonic and/or in-person visits with members receiving home care services
• Assess member needs and authorize appropriate services Create and maintain member care plans
• Monitor services and evaluate effectiveness of interventions
• Promote integration of services for members to enhance continuity of care
• Complete assigned work plan objectives and projects in a timely manner
• Attend regular staff meetings and participate in Interdisciplinary Care Team meetings

Education Qualification: -

• RN/LSW/LISW license required in Ohio
• Minimum of 1 year of case management or managed care experience
• Minimum of 1 year working with individuals with chronic conditions and home care supports

Required Skills: -

• Strong assessment skills
• Excellent organizational skills
• Ability to work independently
• Comfort working with individuals with chronic conditions
• Proficiency in using a laptop; monitors recommended but not required
