About the Role:
- We are seeking a Case Manager to work remotely with home visits required in the Greater Columbus, OH area.
- The role involves assessing member needs, authorizing services, creating and maintaining care plans, and monitoring services.
Responsibilities: -
- Conduct telephonic and/or in-person visits with members receiving home care services
- Assess member needs and authorize appropriate services Create and maintain member care plans
- Monitor services and evaluate effectiveness of interventions
- Promote integration of services for members to enhance continuity of care
- Complete assigned work plan objectives and projects in a timely manner
- Attend regular staff meetings and participate in Interdisciplinary Care Team meetings
Education Qualification: -
- RN/LSW/LISW license required in Ohio
- Minimum of 1 year of case management or managed care experience
- Minimum of 1 year working with individuals with chronic conditions and home care supports
Required Skills: -
- Strong assessment skills
- Excellent organizational skills
- Ability to work independently
- Comfort working with individuals with chronic conditions
- Proficiency in using a laptop; monitors recommended but not required
