This role is for an Ohio-licensed RN who will support members with chronic or complex health conditions by coordinating their care.
Qualifications:
- RN licensed in Ohio
- Lives in Clark County, OH area
- Has at least 1 year of case management, managed care experience
- Comfortable working with chronic conditions and home care services
- Okay with home visits.
