Responsibilities:
- Ensures patients follow up with mental health or substance use treatment after hospital or ER visits.
- Help meet HEDIS quality measures (basically healthcare quality tracking)
- Provide ongoing care support and guidance
Qualifications:
- Active behavioral health license (LCSW/LISW, LPC/LPCC/LMHC, LMFT, or CADC/SUDP)
- NPI number
- Substance use treatment
- Care management, or related clinical settings
