Job Title: Social Worker (MSW) – Acute Inpatient Case Management

Location: Detroit, Michigan. 

Duration: 12-Month Contract (Initial 13-week commitment required)

Pay Range: $41–$50/hr 

**About the Role**

We are seeking an experienced Social Worker to join our team on a 13-week contract. The ideal candidate will have extensive experience in case management, particularly in acute inpatient settings, and will be responsible for providing direct social work services to students and families. This role requires a quick adaptation to the environment with limited on-site training.

**Responsibilities**

• Provide direct social work services to students and families, addressing personal and environmental issues related to social challenges and lifestyle changes in a home care setting.
• Collaborate with students, families, and school staff to develop effective plans that meet the needs of pediatric/adolescent students during school and occasionally outside of school.
• Implement the five phases of the social work process to deliver services to pediatric/adolescent student populations.
• Offer referrals to community resources and agencies.
• Communicate effectively with students, families, and school staff to promote patient satisfaction and achieve goals.
• Coordinate student care with other school staff members to ensure desired outcomes, quality, and efficient resource utilization.

**Education Qualification**

• Completion of a Master's Degree in Social Work from an accredited program.

**Required Skills**

• A minimum of 5 years of case management experience, with at least 2 years of recent acute inpatient case management experience.
• Discharge planning experience.
• Knowledge of pediatric and adolescent patient care.
• Previous home care experience.
• Current licensure as a Social Worker in the State of Michigan: Masters Social Worker license (LMSW) or Limited Masters Social Worker License (LLMSW).
• Valid Michigan driver’s license and a dependable vehicle insured as required by the State of Michigan.
