Qualifications:
- Active RN/LMSW/LLMSW/LBSW license
- MI Choice or HCBS Waiver programs
- Background in care management/LTSS/Chronic Conditions
- Reliable transportation + insurance (≈50% travel within Wayne County or Macomb County)
