  • Need bilingual candidate who speak Chinese or Mandarin or Cantonese.
  • License acceptable Registered Nurse or Licensed Clinical Social Worker or Licensed Master Social Worker
  • Experience with Uniform Assessment System and Case Manager or Case Management or Care Manager or Care Management or Home Care.
