  • Seeking a Case Manager to work with members, providers, and multidisciplinary team members to assess, plan, and coordinate integrated care delivery for members with high need potential. 

Qualification: 

  • One or two years of experience working with Case Management.
  • Worked with Behavioral health and long term care.

Education:

  • Bachelor's or Master's Degree (preferably in a social science, psychology, gerontology, public health or social work or related.
