About the Role:

• We are looking for a long-term care case manager in the Port Charlotte area.
• The position will be remote with the requirement to conduct face-to-face visits in the member's home, ALF, or SNF.
• The ideal candidate should have a BA in related fields like social work or health administration.

Responsibilities: -

• Identify services that members are eligible to receive
• Schedule appointments every quater or as needed
• Coordinate transportation for members if needed
• Identify gaps in care or health care needs 
• Access resources for special health care needs and assist caregivers with day-to-day stress -
• Coordinate transitions between different settings, including hospital discharge
• Assess eligibility for long-term care services and support
• Connect members with community resources
• Coordinate additional services like physical therapy or "Meals on Wheels" with providers
• Collaborate with primary care providers, family members, caregivers, and other providers

Education Qualification: -

• BA in related fields like social work or health administration

Required Skills: -

• Strong communication and interpersonal skills - Ability to assess member needs and develop care plans
• Knowledge of community resources and social services programs
• Ability to work collaboratively with a multidisciplinary team
• Strong organizational and time management skills
• Proficiency in documenting care plan progress
• Ability to conduct face-to-face or home visits as required
• Dependable, reliable, and able to maintain productivity and quality measures
• Willingness to attend regular staff meetings and participate in Interdisciplinary Care Team (ICT) meetings
• Ability to assist in the orientation and mentoring of new team members
