About the Role:

• We are looking for a long-term care case manager in the Lee County (Cape Coral or Ft. Myers area) to work remotely but conduct face-to-face visits in the member's home, ALF, or SNF. The ideal candidate should have a BA in related fields like social work or health administration.

Responsibilities: -

• Identify eligible services for members
• Schedule appointments every or as needed
• Coordinate transportation for members if necessary
• Identify gaps in care and healthcare needs
• Access resources for special healthcare needs and assist caregivers with daily stress
• Coordinate transitions between different settings, including hospital discharge
• Assess eligibility for long-term care services and support
• Connect members with community resources
• Arrange services with primary care providers, family members, caregivers, and other providers
• Conduct face-to-face or home visits as required

Education Qualification: -

• BA in related fields like social work or health administration

Required Skills: -

• Strong communication and interpersonal skills 
• Ability to assess member needs and develop care plans
• Knowledge of community resources and social services programs
• Ability to work collaboratively with members, caregivers, and healthcare professionals 
• Proficient in documenting care plan progress
