  • About the Role We are looking for a long-term care case manager  to work remotely but conduct face-to-face visits in the member's home.
  • The ideal candidate should have a BA in related fields like social work or health administration.
  • Responsibilities Schedule appointments as needed.
  • Coordinate transportation for members if necessary
  • Identify gaps in care and healthcare needs
  • Access resources for special healthcare needs and assist caregivers with daily stress.
  • Coordinate transitions between different settings, including hospital discharge.
  • Arrange services with primary care providers, family members, caregivers, and other providers.
  • Conduct face-to-face or home visits as required.
  • Education Qualification BA in related fields like social work or health administration.
