100% REMOTE:: Pay range:: $40/hr. to $42/hr.
 
Responsibilities:
  • Assess patient's clinical needs against established guidelines to determine appropriate level of care.
  • Evaluate the necessity, appropriateness, and efficiency of medical services provided.
  • Coordinate and assist in implementing care plans for members.
  • Monitor and coordinate services provided outside of the network, negotiate fees when necessary.
  • Coordinate delivery of high quality, cost effective care supported by clinical practice guidelines.
  • Monitor patient's medical care activities and outcomes for appropriateness and effectiveness.
  • Advocate for members/families to coordinate resource utilization and evaluate services provided.
  • Encourage member participation and compliance in case/disease management programs.
  • Document accurately and comprehensively based on standards of practice and organization policies.
  • Interact and communicate with multidisciplinary teams to ensure continuity and efficiency of care.
  • Understand fiscal accountability and its impact on resource utilization.
  • Evaluate care, analyze variances, and participate in quality improvement programs.
  • Complete other assigned functions as requested by management.
 
Education Qualification:
  • Associate's or bachelor's degree in nursing or a health-related field OR accredited diploma nursing school.
  • Minimum of two years of clinical experience.
  • Active New Jersey Registered Nurse License.
 
Required Skills:
  • Proficiency in personal computers and software in a Windows environment.
  • Knowledge of case/care management principles.
  • Understanding of utilization management principles.
  • Basic knowledge of health care contracts and benefit eligibility requirements.
  • Familiarity with hospital structures and payment systems
