                          Must reside in Dona Ana County, NM                           

  • Assess and develop care plans for members. 
  • Conduct phone, home, or in-person visits.
  • Coordinate care with healthcare professionals.
  • Monitor and update care plans regularly.
  • Support members with behavioral health and wellness programs.

Requirements:

  • Certifications: LCSW, CCM, LPC, LMFT, CHEP, APSW, or Respiratory Therapist.
  • Experience: 1-2 years in case management, disease management, managed care, or medical/behavioral health.
  • License: Valid driver's license with reliable transportation.

Education: Bachelor's or Master's in social sciences, psychology, public health, social work, or related field.
