Responsibilities:

• We are seeking a Case Manager based in Virginia who will work remotely and in the field to support our SMI (Severe Mental Illness) Medicaid Population.
• The Case Manager will conduct face-to-face assessments at members homes, participate in interdisciplinary care team meetings, and ensure care plans are tailored to member's needs.
• Complete clinical assessments of members and determine eligibility for case management.
• Develop and implement case management plans in collaboration with members, caregivers, and healthcare professionals.
• Conduct telephonic, face-to-face, or home visits as required.
• Monitor care plans, document interventions, and suggest changes as needed.
• Local travel of up to 40% may be required which will be reimbursed.

License:

• Must be a licensed for the state of Virginia Required licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Case Manager (CCM), Certified in Health Education and Promotion (CHEP), Licensed Professional Counselor (LPC/LPCC), Respiratory Therapist, or Licensed Marriage and Family Therapist (LMFT)

Pay Range: $49/Hour
